All tests, procedures, consultations, or related healthcare services must be booked at least three (3) days in advance.

Nexvita does not provide healthcare services directly and solely facilitates information, appointment bookings, and discounted access to healthcare services.

Nexvita shall not be held responsible for any outcomes arising from services availed through healthcare providers.

Nexvita does not guarantee:

• Service timelines
• Availability of healthcare providers
• Quality or outcome of services

Any regulatory authorization, license, certification, or compliance status of healthcare service providers is not independently verified, validated, or cross-checked by Nexvita.

Nexvita shall not be responsible for any malpractice, negligence, misconduct, or regulatory violations committed by healthcare service providers.

Annual Health Plans

Annual Health Plans may be cancelled within ten (10) days from the date of purchase only if the following conditions are fulfilled:

1. Nexvita is unable to provide discounts on a specific test or treatment in the customer's city.
2. Discounts can only be provided at a new medical center where the customer has never previously visited or where no ongoing treatment exists. Discounts shall not be applicable at medical centers where the customer has already visited or is currently undergoing treatment.
3. Nexvita will make reasonable efforts to arrange discounts at up to three (3) medical centers. If Nexvita is unable to provide at least a 10% discount on published rates, the customer may become eligible for cancellation and refund consideration.
4. The customer must submit a cancellation request via email to: info@nexvita.in
   The request must include a valid reason for cancellation.

After ten (10) days from the date of purchase, the Health Plan shall become strictly non-cancellable and non-refundable.

If a cancellation request is approved, only the "Plan Cost" component shall be refunded. Under no circumstances shall the following charges be refundable:

• Processing Cost
• Payment Gateway Charges

The Health Plan cost structure consists of:

Nexvita does not claim ownership of any material, content, reviews, comments, ratings, images, videos, or information submitted by users through the Site.

• Treatment outcomes or results

Nexvita shall not be responsible for:

• Quality of healthcare centers or services
• Treatment costs or billing disputes
• Outcomes or consequences of treatment

Nexvita only assists in connecting patients with healthcare centers.

20. Survival of Terms After Agreement Ends

These Terms of Use and the relationship between you and Nexvita shall be governed by the laws of India, without regard to conflict of law principles.

You and Nexvita agree to submit to the exclusive jurisdiction of the courts located in New Delhi, India for any dispute arising in connection with these Terms of Use or the Site.

Failure by Nexvita to act against any breach shall not constitute waiver of its rights against subsequent or similar breaches.

Except as expressly stated, no third party shall have any rights or benefits under these Terms of Use.

These Terms of Use constitute the complete and entire agreement between you and Nexvita regarding use of the Site and supersede all prior agreements or understandings.

1. Employees, partners, and associated staff of Nexvita shall not be held responsible for any loss, harm, or damage arising from the use of information available on the portal. Users are advised to exercise their own discretion before availing any services.
2. Nexvita Health Plan is not an insurance policy. It is a healthcare assistance and facilitation service intended to assist users with bookings, rates, and discounts wherever applicable.
3. Users may independently evaluate whether the Nexvita Health Plan complements their insurance policy and are advised to consult their insurance provider for the same.
4. All facts, figures, and information displayed on the Site are approximate in nature and represent estimates or viewpoints of the Nexvita team. Users are advised to conduct independent research before using Nexvita services.
5. Rates, offers, discounts, and charges may change at any time without prior notice, and Nexvita shall not be responsible for such changes.
6. Users are advised to conduct an independent market survey before purchasing the Nexvita Health Plan. Membership purchase shall be deemed voluntary and made with full consent, and the user agrees to indemnify Nexvita accordingly.
7. Nexvita will make reasonable efforts to provide discounts and benefits to Health Plan members wherever possible. However, such discounts are subject to arrangements with healthcare centers and may vary from case to case.
8. Healthcare centers displayed on the Site may not always remain empaneled due to changing business arrangements. Members are advised to contact Nexvita via email to verify the latest status of any healthcare center and available discounts.
9. Location details and map information of healthcare centers displayed on the Site may not always be accurate. Users are advised to contact the healthcare center directly for exact location details. Nexvita shall not be liable for location inaccuracies.
10. Nexvita will make reasonable efforts to arrange services at the healthcare center preferred by the customer. If not feasible, Nexvita may assist the customer with an alternative empaneled healthcare center.
11. Nexvita does not guarantee or assume responsibility for the quality, management, capability, or performance of any healthcare center. All healthcare centers operate independently, and users avail services entirely at their own risk.
12. Nexvita shall not be responsible for any commitments, promises, or representations made by agents or third parties. Customers are strongly advised to review all Nexvita policies carefully before purchasing any Health Plan.
13. The list of healthcare centers available on Nexvita.in is dynamic in nature and subject to additions, removals, or modifications from time to time.
14. Booking an appointment through Nexvita constitutes a request and authorization for Nexvita to assist in arranging appointments as per customer convenience. Final confirmation shall remain subject to the availability and discretion of the healthcare center and its practitioners.
15. Customers must generate a booking code before visiting or contacting any healthcare center. Healthcare centers may deny discounts or benefits if the booking code received via SMS is not presented.
16. Nexvita discourages "No Show" behavior after appointment booking. In case of a no-show, the user may be restricted from making further bookings for a period of four (4) months.

18. The terms "Health Center(s)" and "Medical Center(s)" shall have the same meaning and may be used interchangeably throughout this document.

Nexvita assists customers in obtaining discounts on healthcare bills, which may apply to healthcare expenditures of unlimited value, subject to Nexvita's understanding and arrangements with the respective healthcare service provider.

The customer shall be responsible for payment of the remaining balance amount after the applicable discount has been applied.

POLICY FOR GUARANTEED SAVINGS

Nexvita Plan holders may avail guaranteed savings subject to the following conditions:

1. Tests, treatments, surgeries, or healthcare services must be booked at least three (3) days in advance at Nexvita empanelled healthcare centers only.
2. Nexvita reserves the right, at its sole discretion, to change the selected healthcare center and provide services through an alternative empanelled healthcare center depending upon prevailing circumstances and arrangements at the time of booking.

4. Guaranteed savings shall apply only if the patient's total bill value at empanelled healthcare centers, after applying the Nexvita discount code, satisfies the applicable eligibility criteria.
